Acne usually begins in puberty or prepuberty as your hormones begin to shift, and may arise again or worsen during the hormonal fluctuations of perimenopause and menopause. When your body produces more androgens — “male” hormones such as testosterone — your skin secretes an excess of the waxy oil known as sebum.
Excessive sebum clogs your pores so that your skin can’t shed dead cells at a regular rate. Instead, the sebum and dead cells get stuck in the pore, where they irritate your skin and erupt into lesions such as blackheads and whiteheads, also known as comedones.
The excess sebum provides the perfect food for a bacterium — Propionibacterium acnes (P acnes) — that lives on your skin. As the bacteria feed on the sebum that’s clogged in your follicle and begin to reproduce, they cause an inflammatory reaction that may result in a pimple or cyst.
Although you may be bothered by the look of comedones, pimples, and cysts, it’s important to remember that acne actually originates underneath your skin, not on the surface.
